About the Opportunity

Katherine West Health Board (KWHB) is currently looking for a Prevention and Population Health Nurse to join their team for an opportunity in the Katherine West region in the NT. This position is well suited to someone with substantial clinical experience. 

 

The Prevention and Population Health Nurse (PPHN) will work with community based Primary Health Care Teams to provide, plan and manage care for clients with RHD. The PPHN will also be involved with health promotion and community education, data collection and monitoring, health hardware, clinical services, training and research in the area of treatment and prevention of RHD. 

Under the guidance and support from the Senior Prevention and Population Health Advisor and the Prevention and Population Heath Coordinator, the (PPHN) will also assist as needed in the Healthy Clean Bodies and Homes action plan as well as primary and secondary prevention health issues such as trachoma and post strep glomerulonephritis.

 

Your responsibilities will include (but are not limited to):

  • Support the Prevention and Population Health Coordinator with the maintenance and review of the RHD program across the Katherine West region.
  • Support clients and their families with self-management of RHD.
  • Assist with the development and the implementation of and resources and strategies that aim to increase adherence to secondary prophylactic treatment.
  • Provide clinical leadership to health centre staff in relation to identification and treatment of rheumatic fever, rheumatic heart disease, post strep glomerulonephritis and throat and skin infections identified in the population within the Katherine West region.
  • Assist with other preventative and population health activities such as immunisations, trachoma and anaemia treatment.
