Summary of Position
The Punyu Yirdipirrki (Good Kidneys) Care Coordinator (PYCC) is an Aboriginal Health Practitioner or Registered Nurse educated and authorised to function autonomously and collaboratively in a clinical
role in chronic kidney disease (CKD) care, with an understanding of Aboriginal cultural values, and commitment to the advancement and self-determination of Aboriginal communities.

The PYCC position promotes excellence in clinical standards, client-centred care planning and continuity of care as clients move between health services.

The PYCC will work closely with the project team, in a collaborative enviroment, working alongside the Chronic Disease and Aboriginal Health Coaches, to implement and embed a regional model of integrated renal care coordination, including use of the Territory Kidney Care (TKC) data platform, across the Katherine West region.

Position Description

Your responsibilities will include (but are not limited to):

• Provide high quality, individualised and culturally safe care coordination for clients and their families living with, or at risk of, chronic kidney disease (CKD), across early-stage, advancedstage
  and supportive care.
• Work collaboratively with all health centre, specialist and tertiary staff to ensure appropriate planning and implementation of care for clients with CKD, including timely referral to nephrology
  and dialysis services.
• Use the Territory Kidney Care (TKC) platform to support active monitoring, recall and communication for CKD clients, and contribute to the development of region-specific templates
  and clinical alerts.
• Provide advanced clinical leadership to nurses, Aboriginal Health Practitioners, and allied health professional staff in renal and chronic disease care.
• Coordinate and lead case conferencing for complex CKD clients within a multidisciplinary team of primary and tertiary health care staff.
• Support consumer and community engagement activities, including social days and communityconsultation, to inform improvements in regional renal care coordination.
